- Bring your insurance card, cash or check
- Wear a short sleeved shirt as vaccine will be given in the arm
- Due to close contact, masks are required
- At La Plata School’s request, flu vaccine will not be given at school this year. Vaccine available for students ages kindergarten – 12th grade. Under 18 must have a parent or guardian signature
- Vaccine available for adults
- Vaccine available for 6 month – pre-k at our office
